National Healthcare Group looks to digital records for community care

National Healthcare Corporation’s community care partners in Singapore have been provided access to a digital platform to manage health programmes and have the ability to create digital records.

WHAT IT’S ABOUT

Recently, NHG has partnered with credential platform provider Accredify to develop the NHG Cares Partner Portal.

According to the press release, the portal simplifies the management of health programs, automatically synchronized with NHG.‘s health and social care marketplace, Health Kampung. It allows NHG community partners to monitor demand by tracking registration and attendance and updates in real time.

Through the portal, NHG partners can also access and leverage insights from anonymized public health data to design community-relevant programs.

In addition, NHG partners can issue electronic certificates to participants to recognize their efforts in health and fitness management, thereby encouraging self-health management in the community.

NHG residents can then access general care services through the portal, including social screening and prescribing services.

The portal now has more than 20 community partners on board and has automated more than 400 health plans since its launch in April.

WHY IT MATTERS

NHG empowers people in the community to take better control of their health by bringing community care resources closer to them.

“The [p]“Ortal empowers our community partners to effectively reach out to people and guide them towards better health and well-being through tailored programmes,” said Ong Jing Fang, Head of Population Health at NHG.

Accredify also shared that it has the ability to introduce verifiable healthcare records through its online portal. “The NHG Cares Partner Portal creates the infrastructure needed to enhance continuity of care and individual ownership of health information,” said CEO and co-founder Quah Zheng Wei.

THE BIGGER TREND

NHG has also deployed the Next Generation EMR (NGEMR) system as part of its digital transformation. The system is gradually being rolled out across the cluster, starting with National Healthcare Group Polyclinic in 2021. Tan Tock Seng Hospital then rolled out the system the following year, becoming the first hospital in the cluster to go live with the integrated EMR system. Developed by Synapxe, the NGEMR is also being rolled out across the cluster. National University Hospital System Cluster.
