Michigan suppliers collaborate on SDOH interoperability supporting HIE



Organized by the nonprofit Michigan Health Information Network (MiHIN), the statewide health information exchange, the new Community of Collaborative Practice focuses on creating a conduit for referral data to flow. interactions between platforms.

Eight platform providers serving healthcare and social organizations in Michigan have begun work on strategic data governance to create a trusted mechanism, with the right safeguards in place. , to facilitate the exchange of health care information to improve each person’s overall care coordination.

The Community of Practice seeks to assist HIE in eliminating information silos. CoP’s initial focus was on creating a path for referral data to flow interactively between platforms to support its shared mission of supporting community-led systems of care.

One challenge the collaborators will tackle is data flow with some social care organizational systems that challenge technical feasibility.

Care Advisors, Findhelp, PCE Systems, Riverstar, Unite Us and WellSky, which have previously committed to working together to facilitate data aggregation to inform state policies and quality assessments, have been approved by CareConvene and Holon joined to form the CoP.

Although Michigan’s HIE will act as a trusted health data exchange broker, service providers may use or maintain an electronic health record system of their choice.
